HOSPICE & HOME HEALTH REGISTERED NURSE
FULL TIME HOSPICE / Float Team Tues-Sat 8a-4:30p FULL TIME HOSPICE/ Central Team Mon-Fri 8a-4:30p FULL TIME HOME HEALTH /West Team Mon-Fri 8a-4:30p PRN HOSPICE & HOME HEALTH / Weekdays & Weekends Home Visits Cleveland East & West Side **PRN Nursing is Great for Spring & Summer Extra $$$$
Summary:
Provides direct or indirect skilled comprehensive nursing care to a specified (or variety) of age and diagnostic groups.
Qualifications :
- Completion of an accredited RN education program with a diploma, Associate’s or Bachelor's degree. Bachelor's degree in Nursing preferred.
- 1 year experience in medical-surgical nursing.
- Hospice experience required for Hospice RNs
- Experience in home health care or community nursing.
- Experience working as part of a team and working effectively with persons and communities from diverse cultural, social, and ethnic backgrounds. The ability to work in a complex institutional structure requiring significant collaboration.
- Ability to work independently with minimal supervision.
Required Licenses, Registrations and/or Certifications:
- Registered Nurse, licensed in the State of Ohio.
- CPR certification is required.
- Valid driver's license.
- Hospice requires initial certification by National Board Certification for Hospice and Palliative Nurses (NBCHPN) within 1 year of employment and maintenance of certification thereafter
Key Essential Functions:
- Hospice:
Demonstrates expertise in pain management techniques, including assessing pain and adjusting of pharmacological and non-pharmacological medications/therapies to achieve effective pain control.
- Demonstrates knowledge of disease progression and appropriate palliative care measures for symptom control. Able to distinguish between curative, preventative and symptomatic palliative treatments and therapies.
- Provides emotional, psychological, spiritual support to patient/family during the course of illness and facilitates appropriate and timely referrals to other team members as defined by the Plan of Care.
- Demonstrates knowledge and understanding of the death and dying principles and the grieving process.
- Explains processes, procedures and treatments to patient to gain cooperation, understanding, and ease apprehension.
- Home Health:
- Utilizes principles of the nursing process to assist in data collection, planning and evaluation of nursing care.
- Demonstrates nursing knowledge and skills in the implementation of prevention, therapeutic and rehabilitative nursing care.
- Utilizes effective interviewing techniques; sets limits, and collects data based on specific role.
- Communicates and collaborates effectively with health care providers, third parties, patients and families based on specific role.
- Explains processes, procedures and treatments to patient to gain cooperation, understanding, and ease apprehension.
- Maintains awareness of comfort and safety needs of patient.
- Observes patient, records significant conditions and reactions, and notifies supervisor or Physician of patient's condition and reaction to drugs, treatments, and significant incidents.
- Responds to life saving situations based upon nursing standards, policies, procedures, and protocol.
- Uses oral and written communication to convey pertinent information to members of the health care team in a timely manner.
